0345(5)(b) - at the time of the visit, licensor observed the doors in the immediate area to be opened without a way to notify provider that a child had left the area. 0345(5)(c) - Licensor observed a child to be playing in the outdoor space alone. Licensor made provider aware of the child playing outdoors alone. TA was provided on increasing supervision.
